Form Page

The Form page is the first part of filing through the Filing Wizard. Some information on this page is prepopulated from project information specified in CDM. See SEC Filing Wizard for more information prior to beginning your filing.

To review and complete the Form information:

  1. In the Filing Date section, review the Period End Date, Click on the date to modify it if needed. Date formats entered are converted to the EDGAR format, mm-dd-yyyy. For many form types, using a future date may result in an error (see EDGAR Errors). The date must be between 01-01-1980 and 12-31-2050.
  2. In the Form Type section, review the selected form type. Change the form type if needed by clicking on a form type in the list.
  3. In the Submission Format field, select either Inline or Traditional HTML and XBRL. When the Enable Inline XBRL Tagging option of the project was selected in CDM, this field defaults to Inline.
  4. Click Next.
  5. For some form types, such as 8-K and 8-K/A, you need to specify the following additional options:
    1. Click the Item drop-down arrow to select one or more items.
    2. Click the Item Submission Type drop-down arrow to make a selection.
  6. Click Next to proceed to the Files page.
